It provides a comprehensive explanation of the issues I encountered and the solutions I implemented during my six years and eight months at the National Health Insurance Administration (NHIA). The content covers the following topics:<\/p>\n<ol data-start=\"643\" data-end=\"1502\">\n<li data-start=\"643\" data-end=\"741\">The Global Budget System, Point System, and Premium Rates of National Health Insurance (NHI).<\/li>\n<li data-start=\"742\" data-end=\"850\">Key Pain Points of Taiwan\u2019s NHI, Chronic Diseases, Non-Chronic Diseases, and Unhealthy Life Expectancy.<\/li>\n<li data-start=\"851\" data-end=\"980\">The Healthcare Payment System, Physician Specialization Choices, Nursing Workforce, and NHI Coverage for Overseas Residents.<\/li>\n<li data-start=\"981\" data-end=\"1062\">The Establishment of the NHI Cloud-Based Medical Information Sharing System.<\/li>\n<li data-start=\"1063\" data-end=\"1146\">Hierarchical Medical Referral System and Cost-Sharing Mechanisms for Patients.<\/li>\n<li data-start=\"1147\" data-end=\"1269\">Pharmaceutical Benefits, Next-Generation Genetic Testing, and the Role of Private Insurance in Coordination with NHI.<\/li>\n<li data-start=\"1270\" data-end=\"1314\">The Role of Fraud Investigation in NHI.<\/li>\n<li data-start=\"1315\" data-end=\"1502\">My Lifelong Commitment to Medicine, including Advancing Kidney Transplantation and Promoting Organ Donation.<br data-start=\"1426\" data-end=\"1429\" \/><strong data-start=\"1429\" data-end=\"1442\">Epilogue:<\/strong> Persisting with a Calm Mindset in Doing the Right Things.<\/li>\n<\/ol>\n<p data-start=\"1504\" data-end=\"2072\">I understand that Taiwan has approximately <strong data-start=\"1547\" data-end=\"1568\">54,000 physicians<\/strong>, <strong data-start=\"1570\" data-end=\"1588\">200,000 nurses<\/strong>, and numerous public health and social science scholars, all of whom have extensive discussions on NHI. However, gaining a thorough understanding of the system\u2019s practical operations requires time and study. Through professional charts and easy-to-understand explanations, this book aims to help medical professionals, public health scholars, hospital administrators, and the general public better understand Taiwan\u2019s NHI and grasp the direction of future health insurance reforms.<\/p>\n<p data-start=\"2074\" data-end=\"2574\">I was born in <strong data-start=\"2088\" data-end=\"2121\">Huwei Township, Yunlin County<\/strong>, and I still vividly remember my childhood experiences with healthcare. When I was in kindergarten, I had a high fever in the middle of the night, and my parents carried me to <strong data-start=\"2298\" data-end=\"2336\">Dr. Lee\u2019s internal medicine clinic<\/strong> for treatment. In elementary school, while washing dishes for my mother, I accidentally cut my wrist and had to be taken to <strong data-start=\"2461\" data-end=\"2491\">Dr. Yang\u2019s surgical clinic<\/strong> for stitches. These experiences with local doctors left a deep impression on me.<\/p>\n<p data-start=\"2576\" data-end=\"3502\">In <strong data-start=\"2579\" data-end=\"2587\">2015<\/strong>, Tainan faced its first large-scale <strong data-start=\"2624\" data-end=\"2649\">dengue fever outbreak<\/strong>, with the majority of patients flooding into <strong data-start=\"2695\" data-end=\"2752\">Cheng Kung University Hospital\u2019s emergency department<\/strong>. At that time, Deputy Minister of Health and Welfare <strong data-start=\"2806\" data-end=\"2823\">Lin Tzou-Yien<\/strong> instructed me to immediately designate two inpatient wards at <strong data-start=\"2886\" data-end=\"2905\">Tainan Hospital<\/strong> to accommodate dengue fever patients. We first posted a notice advising non-severe cases to avoid seeking treatment at <strong data-start=\"3025\" data-end=\"3044\">Tainan Hospital<\/strong> and then further instructed that non-dengue fever patients should not go there. This significantly alleviated the burden on <strong data-start=\"3169\" data-end=\"3203\">Cheng Kung University Hospital<\/strong>. Patients with mild fever and discomfort received conservative treatment at <strong data-start=\"3280\" data-end=\"3299\">Tainan Hospital<\/strong>, while those experiencing <strong data-start=\"3326\" data-end=\"3384\">respiratory distress or requiring ICU and ECMO support<\/strong> were transferred to <strong data-start=\"3405\" data-end=\"3439\">Cheng Kung University Hospital<\/strong>. This was a highly successful example of <strong data-start=\"3481\" data-end=\"3499\">medical triage<\/strong>.<\/p>\n<p data-start=\"3504\" data-end=\"3648\">In <strong data-start=\"3507\" data-end=\"3515\">2018<\/strong>, to implement <strong data-start=\"3530\" data-end=\"3559\">hierarchical medical care<\/strong>, NHIA actively promoted <strong data-start=\"3584\" data-end=\"3629\">vertically integrated strategic alliances<\/strong>, which included:<\/p>\n<ul data-start=\"3649\" data-end=\"3956\">\n<li data-start=\"3649\" data-end=\"3694\"><strong data-start=\"3651\" data-end=\"3669\">Taipei Region:<\/strong> 16 strategic alliances<\/li>\n<li data-start=\"3695\" data-end=\"3757\"><strong data-start=\"3697\" data-end=\"3717\">Northern Region:<\/strong> 13 hierarchical medical care projects<\/li>\n<li data-start=\"3758\" data-end=\"3804\"><strong data-start=\"3760\" data-end=\"3779\">Central Region:<\/strong> 18 strategic alliances<\/li>\n<li data-start=\"3805\" data-end=\"3852\"><strong data-start=\"3807\" data-end=\"3827\">Southern Region:<\/strong> 17 strategic alliances<\/li>\n<li data-start=\"3853\" data-end=\"3910\"><strong data-start=\"3855\" data-end=\"3885\">Kaohsiung-Pingtung Region:<\/strong> 14 strategic alliances<\/li>\n<li data-start=\"3911\" data-end=\"3956\"><strong data-start=\"3913\" data-end=\"3932\">Eastern Region:<\/strong> 3 strategic alliances<\/li>\n<\/ul>\n<p data-start=\"3958\" data-end=\"4146\">A total of <strong data-start=\"3969\" data-end=\"3991\">24 medical centers<\/strong> and <strong data-start=\"3996\" data-end=\"4021\">82 regional hospitals<\/strong> served as leading institutions, integrating <strong data-start=\"4066\" data-end=\"4092\">302 district hospitals<\/strong> and <strong data-start=\"4097\" data-end=\"4127\">6,600 primary care clinics<\/strong> into the system.<\/p>\n<p data-start=\"4148\" data-end=\"5096\">When people fall ill, they naturally want to receive treatment from the <strong data-start=\"4220\" data-end=\"4236\">best doctors<\/strong>. <strong data-start=\"4238\" data-end=\"4284\">National Taiwan University Hospital (NTUH)<\/strong>, as the leading medical institution, often has patients lining up early in the morning to secure an appointment. To address this, NTUH launched the <strong data-start=\"4433\" data-end=\"4456\">\u201cStar-Moon Project\u201d<\/strong>, collaborating with primary care clinics to enhance <strong data-start=\"4509\" data-end=\"4540\">two-way referral mechanisms<\/strong>. Under this system, if a primary care clinic determines that a patient needs further evaluation at a <strong data-start=\"4642\" data-end=\"4660\">medical center<\/strong>, they <strong data-start=\"4667\" data-end=\"4702\">no longer need to queue at dawn<\/strong>. Instead, they can go to NTUH\u2019s <strong data-start=\"4735\" data-end=\"4752\">referral desk<\/strong> with a <strong data-start=\"4760\" data-end=\"4777\">referral slip<\/strong> and secure one of the hospital\u2019s reserved outpatient slots. After appropriate examination and treatment, follow-up care is provided at the <strong data-start=\"4917\" data-end=\"4936\">original clinic<\/strong>, ensuring <strong data-start=\"4947\" data-end=\"4969\">continuity of care<\/strong>. Many of the <strong data-start=\"4983\" data-end=\"5006\">strategic alliances<\/strong> now follow this <strong data-start=\"5023\" data-end=\"5043\">two-way referral<\/strong> model to facilitate <strong data-start=\"5064\" data-end=\"5093\">hierarchical medical care<\/strong>.<\/p>\n<p data-start=\"5098\" data-end=\"6054\">Primary care physicians undergo <strong data-start=\"5130\" data-end=\"5151\">rigorous training<\/strong> in medical centers and regional hospitals, and their capabilities are <strong data-start=\"5222\" data-end=\"5241\">not in question<\/strong>\u2014they provide <strong data-start=\"5255\" data-end=\"5281\">excellent patient care<\/strong>. NHIA\u2019s <strong data-start=\"5290\" data-end=\"5301\">IT team<\/strong> has also <strong data-start=\"5311\" data-end=\"5374\">enhanced the cloud-based medical information-sharing system<\/strong>, allowing primary care physicians to access <strong data-start=\"5419\" data-end=\"5459\">lab test results and imaging reports<\/strong> from medical centers. This <strong data-start=\"5487\" data-end=\"5508\">data transparency<\/strong> is at the core of hierarchical medical care. Since Taiwan already has a <strong data-start=\"5581\" data-end=\"5605\">solid infrastructure<\/strong>, the government must <strong data-start=\"5627\" data-end=\"5660\">actively promote public trust<\/strong> in <strong data-start=\"5664\" data-end=\"5691\">primary care physicians<\/strong>. When necessary, patients will be <strong data-start=\"5726\" data-end=\"5757\">referred to medical centers<\/strong>, preventing delays in <strong data-start=\"5780\" data-end=\"5803\">disease progression<\/strong>. For instance, <strong data-start=\"5819\" data-end=\"5855\">Kaohsiung physician Wang Hung-Yu<\/strong> has shown exceptional dedication to his patients while maintaining <strong data-start=\"5923\" data-end=\"5947\">strong collaboration<\/strong> with local hospitals\u2014this is the <strong data-start=\"5981\" data-end=\"5996\">ideal model<\/strong> of hierarchical medical care that we aspire to achieve.<\/p>\n<p data-start=\"6056\" data-end=\"6848\">I must emphasize that while <strong data-start=\"6084\" data-end=\"6119\">National Health Insurance (NHI)<\/strong> and <strong data-start=\"6124\" data-end=\"6159\">Universal Health Coverage (UHC)<\/strong> share the goal of <strong data-start=\"6178\" data-end=\"6223\">ensuring access to appropriate healthcare<\/strong>, their <strong data-start=\"6231\" data-end=\"6282\">financial structures and operational strategies<\/strong> are fundamentally different. Politicians focus on securing <strong data-start=\"6342\" data-end=\"6374\">more social welfare benefits<\/strong> for their constituents, social scientists advocate for <strong data-start=\"6430\" data-end=\"6448\">social justice<\/strong>, and representatives on the <strong data-start=\"6477\" data-end=\"6494\">NHI Committee<\/strong> are concerned about <strong data-start=\"6515\" data-end=\"6551\">employers&#8217; premium contributions<\/strong>. Meanwhile, healthcare providers continue to <strong data-start=\"6597\" data-end=\"6625\">face excessive workloads<\/strong>. When have we ever heard the public express a willingness to <strong data-start=\"6687\" data-end=\"6733\">pay more for the medical care they receive<\/strong>? Only <strong data-start=\"6740\" data-end=\"6777\">overseas Taiwanese returning home<\/strong> truly realize how <strong data-start=\"6796\" data-end=\"6845\">undervalued healthcare services are in Taiwan<\/strong>.<\/p>\n<p data-start=\"6850\" data-end=\"7034\">Beyond <strong data-start=\"6857\" data-end=\"6900\">reducing unnecessary medical procedures<\/strong> to alleviate the workload of healthcare providers, <strong data-start=\"6952\" data-end=\"6985\">adjusting reimbursement rates<\/strong> is the most <strong data-start=\"6998\" data-end=\"7031\">pragmatic management strategy<\/strong>.<\/p>\n<ul data-start=\"7035\" data-end=\"7627\">\n<li data-start=\"7035\" data-end=\"7115\"><strong data-start=\"7037\" data-end=\"7046\">2017:<\/strong> Adjusted <strong data-start=\"7056\" data-end=\"7085\">1,716 critical care items<\/strong>, totaling <strong data-start=\"7096\" data-end=\"7112\">NT$6 billion<\/strong>.<\/li>\n<li data-start=\"7116\" data-end=\"7291\"><strong data-start=\"7118\" data-end=\"7127\">2020:<\/strong> Rationalized <strong data-start=\"7141\" data-end=\"7184\">hospital and primary care reimbursement<\/strong>, totaling <strong data-start=\"7195\" data-end=\"7214\">NT$12.4 billion<\/strong>, including <strong data-start=\"7226\" data-end=\"7246\">NT$2.273 billion<\/strong> for <strong data-start=\"7251\" data-end=\"7288\">400 complex and severe conditions<\/strong>.<\/li>\n<li data-start=\"7292\" data-end=\"7381\"><strong data-start=\"7294\" data-end=\"7303\">2021:<\/strong> Enhanced <strong data-start=\"7313\" data-end=\"7347\">basic medical service payments<\/strong>, totaling <strong data-start=\"7358\" data-end=\"7378\">NT$8.226 billion<\/strong>.<\/li>\n<li data-start=\"7382\" data-end=\"7627\"><strong data-start=\"7384\" data-end=\"7393\">2023:<\/strong> Planned a <strong data-start=\"7404\" data-end=\"7430\">comprehensive revision<\/strong> of the <strong data-start=\"7438\" data-end=\"7470\">medical reimbursement system<\/strong>, intended to be completed over <strong data-start=\"7502\" data-end=\"7517\">nine months<\/strong>. Unfortunately, I <strong data-start=\"7536\" data-end=\"7575\">was unable to complete this mission<\/strong>, leaving <strong data-start=\"7585\" data-end=\"7624\">health insurance reforms unfinished<\/strong>.<\/li>\n<\/ul>\n<p data-start=\"7629\" data-end=\"8334\">Currently, <strong data-start=\"7640\" data-end=\"7679\">all levels of the healthcare system<\/strong> are dissatisfied with the reimbursement structure. The government must <strong data-start=\"7751\" data-end=\"7788\">engage in transparent discussions<\/strong> with medical professionals across <strong data-start=\"7823\" data-end=\"7861\">all specialties and hospital tiers<\/strong>\u2014reform cannot be dictated solely by <strong data-start=\"7898\" data-end=\"7919\">official mandates<\/strong>. The <strong data-start=\"7925\" data-end=\"7964\">nursing night shift pay controversy<\/strong> serves as a prime example. For years, reimbursement points for <strong data-start=\"8028\" data-end=\"8064\">surgical and invasive procedures<\/strong> have been suppressed, leading to the emergence of <strong data-start=\"8115\" data-end=\"8141\">&#8220;sweatshop hospitals.&#8221;<\/strong> With <strong data-start=\"8147\" data-end=\"8179\">nurses leaving the workforce<\/strong> and <strong data-start=\"8184\" data-end=\"8210\">hospital bed shortages<\/strong>, ensuring <strong data-start=\"8221\" data-end=\"8249\">reasonable reimbursement<\/strong> for <strong data-start=\"8254\" data-end=\"8272\">inpatient care<\/strong> will be <strong data-start=\"8281\" data-end=\"8296\">a key focus<\/strong> in future health insurance reforms.<\/p>\n<p data-start=\"8336\" data-end=\"8957\">We all know that <strong data-start=\"8353\" data-end=\"8397\">having more funding makes reforms easier<\/strong>, but we must also <strong data-start=\"8416\" data-end=\"8432\">spend wisely<\/strong>. Taiwan&#8217;s <strong data-start=\"8443\" data-end=\"8487\">distorted healthcare resource allocation<\/strong> must be addressed. Outpatient care accounts for <strong data-start=\"8536\" data-end=\"8545\">70.3%<\/strong> of <strong data-start=\"8549\" data-end=\"8565\">NHI expenses<\/strong>, while inpatient care comprises only <strong data-start=\"8603\" data-end=\"8612\">29.7%<\/strong>. I deeply appreciate <strong data-start=\"8634\" data-end=\"8662\">Premier Su Tseng-Chang\u2019s<\/strong> decisive support for the <strong data-start=\"8688\" data-end=\"8714\">cost-sharing mechanism<\/strong>, despite opposition from some sociologists. However, the <strong data-start=\"8772\" data-end=\"8816\">lack of co-payments for diagnostic tests<\/strong> and the <strong data-start=\"8825\" data-end=\"8861\">removal of registration fee caps<\/strong> have <strong data-start=\"8867\" data-end=\"8890\">stalled NHI reforms<\/strong>, ultimately <strong data-start=\"8903\" data-end=\"8944\">undermining hierarchical medical care<\/strong> in Taiwan.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Lee Po-Chang \/ Chair Professor, College of Public Health, Taipei Medical University This book, which I co-authored with medical journalist Li Shu-Ren, was published last September.
