During my service in the National Health Insurance Administration, my guiding principle was to ensure that healthcare professionals received fair compensation while the public enjoyed the genuine social welfare promised by universal health insurance.<\/p>\n<p data-start=\"3944\" data-end=\"4218\">From holding a scalpel in the operating room to leading national healthcare policy, I always believed that the best systems had already been designed by our public health predecessors. What remained was to make precise corrections and adjustments suited to Taiwan\u2019s reality.<\/p>\n<p data-start=\"4220\" data-end=\"4459\">Over time, I came to realize that while medical fraud certainly exists and must be addressed, the true force consuming healthcare resources is not illegal fraud. It is the vast amount of unnecessary medical care that is legally reimbursed.<\/p>\n<p data-start=\"4461\" data-end=\"4678\">These services comply with regulations, pass administrative review, and may even be professionally defensible. Yet under the incentives of the payment system, they are repeatedly expanded, duplicated, and accumulated.<\/p>\n<p data-start=\"4680\" data-end=\"4713\">This led me to a deeper question:<\/p>\n<p data-start=\"4715\" data-end=\"4750\"><strong data-start=\"4715\" data-end=\"4750\">What exactly are we paying for?<\/strong><\/p>\n<p data-start=\"4752\" data-end=\"5043\">If the system continuously pays for medical services with limited benefit\u2014or no real benefit to patients\u2014then no matter how much we increase the total budget or adjust point values, healthcare professionals will never receive fair returns, and the healthcare system will never truly improve.<\/p>\n<hr data-start=\"5045\" data-end=\"5048\" \/>\n<h2 data-section-id=\"rt5oo5\" data-start=\"5050\" data-end=\"5082\"><span role=\"text\"><strong data-start=\"5053\" data-end=\"5082\">Key Arguments in the Book<\/strong><\/span><\/h2>\n<h3 data-section-id=\"1qto9k2\" data-start=\"5084\" data-end=\"5146\">1. Taiwan\u2019s NHI problem is not simply \u201cinsufficient money\u201d<\/h3>\n<p data-start=\"5147\" data-end=\"5279\">The real issue is that resources are being spent on services that should not be prioritized, leaving too little for high-value care.<\/p>\n<h3 data-section-id=\"1o9zc63\" data-start=\"5281\" data-end=\"5338\">2. Unnecessary care is not a moral failure of doctors<\/h3>\n<p data-start=\"5339\" data-end=\"5447\">Most unnecessary services arise legally and systematically. The problem lies in incentives, not individuals.<\/p>\n<h3 data-section-id=\"lvl54n\" data-start=\"5449\" data-end=\"5503\">3. From tiered co-payments to user-pays principles<\/h3>\n<p data-start=\"5504\" data-end=\"5593\">Reasonable co-payments can help patients and doctors pause and ask an essential question:<\/p>\n<p data-start=\"5595\" data-end=\"5631\"><strong data-start=\"5595\" data-end=\"5631\">Is this service truly necessary?<\/strong><\/p>\n<h3 data-section-id=\"1w8ks8u\" data-start=\"5633\" data-end=\"5684\">4. Claims review alone cannot solve the problem<\/h3>\n<p data-start=\"5685\" data-end=\"5852\">Traditional post-payment audits cannot detect accumulated patterns of low-value care. Reform must combine payment redesign, incentive reform, and AI-driven governance.<\/p>\n<h3 data-section-id=\"1vje0mk\" data-start=\"5854\" data-end=\"5909\">5. Cooperation between NHI and commercial insurance<\/h3>\n<p data-start=\"5910\" data-end=\"6065\">Commercial insurers also suffer from the rising costs of unnecessary care.
